Please include the following documentation:
  • OCF-1
  • OCF-2 (if complete)
  • All financial documentation you have already collected
  • All IRB calculations and OCF-9's prepared by the insurer.
Please include the following documentation:
  • OCF-1 (REQUIRED)
  • OCF-2 (when available)
  • Copies of Disability policies
  • All financial documentation you have already collected (ie. tax returns, notices of assessment)
  • All IRB calculations and OCF-9's prepared to date.
Please include the following documentation:
  • Complete Tax Returns and Notices of Assessment (personal/corporate) for the 3-5 years prior to the incident date OR CRA Printouts with T4 Slip summary for the same period
  • Financial statements of business (if applicable)
  • Employment file
  • Pension booklet and annual pension statements
  • Future Care cost report
  • Medical / Vocational reports (most current only)
  • Summary of AB payments received to date
  • Any other financial documentation you have collected
